Person-centered services tailored to meet individual needs
We provide the programming and services that people with disabilities need to live life on their own terms. We offer personalized services in both community and in-home settings to support individuals and their families. Available through waivers and other funding sources, our services are designed and delivered by professionals who approach each individual with the respect and compassion they deserve.
Case Management Services
Pinnacle Services offers contract case management services to individuals who receive funding through medical assistance, home and community-based waivers. We offer creative, person-centered solutions that allow families and individuals to know and choose the options that best fit their needs. Our case managers take the time necessary to explain options and advocate for individuals who seek services tailored to their unique needs. Case management services are provided weekdays during business hours and flexibly to accommodate anyone’s schedule.
Consultation Services
Pinnacle Services is thrilled to be a part of Community First Services and Supports (CFSS) as a Consultation Services provider. We offer consultation services to individuals who will switch from traditional PCA or CSG services to CFSS choosing either the agency or budget model of service. Our experienced case management team will build on our knowledge of those, and similar, programs to guide you through the process of completing your support plan; from the first draft through to the final approval. Need revisions? No problem. You can trust our employees will be alongside you to make changes to your plan throughout the year and ensure you have the information you need to make CFSS work for you.
Independent Living Services
We can deliver a variety of Independent Living Services including but not limited to Individualized Home Supports with training, without training and Individualized Home Supports with family training. Our Independence Program offered to those who receive funding through medical assistance, home and community-based waivers and is centered in the individual’s home and in the community. We tailor our services to fit the family culture, to meet the individual’s needs and to be flexible in schedule and design. Homemaker services are available to those with funding through the Ryan White HIV/AIDS program. All Independent Living Services are offered on a schedule convenient to the individual requesting the service.
Employment Services
We help people find meaningful work that aligns with an individual’s skills and aspirations. Working with our community partners, we strive to support individuals at any ability to maintaining independence while becoming more connected with the local community. Employment options such as Employment Development, Exploration and Support Services, can be tailored to an individual’s needs. Our employment specialists will work with the entire care team to find a situation that is safe, supportive and in keeping with an individual’s goals. Employment Services are provided weekday during business hours and flexibly to accommodate anyone’s schedule.
